Abstract
Manipulation is a classical method used to treat musculo-skeletal soreness and pain with its own indications , but the etiologies of aching pain are versatile. It is extremely dangerous to apply manipulation before adequate diagnosis. This is a report of two cases with injury of cervical cord myelopathy by inadequate manipulation. The first patient was 52 year-old man who suffered form neck and right hand soreness for move than a year. He received manipulation two weeks before this episode. He had a sense of electrical shock during manipulation with numbness and weakness of all the extremities developed gradually after he returned home. He became disabled from walking later on. The CT scan of cerival spine showed a tumor located at C7 level. Post-op pathological report was neurilemmoma. The patient could ambulate well with walker after rehabilitation. The second patient was a 32 year old man with the chief complaint of neck soreness, left side weakness and urinary retenion developed after manipulation. The CT spine and x-ray showed. He received surgery and rehabilitation program with good recovery. From the experience of these two cases, we did learn that manipulation is a king of treatment need for a clear and unambiguous statement of principles, chaims and limitations. It will be very dangerous in the application manual therapy to spine by non-medical chiropractors who were not trained in anatomy or neuroanatomy. The undesirable side effects and complications should be avoided.
