Abstract

In recent years, the upgrading of both quality of medical care and standard of living in Taiwan has resulted in the rapid growth of elderly population. The care of such diseases associated with aging as cerebrovascular disease, senile dementia, cardiovascular disease and degenerative neurological changes has thus became major medical and social problems in our country. Those elderly individuals who suffer from the diseases mentioned above, usually require bedrest and consequently lose their selfcare abilities. The demand for caregivers, which is a relatively big burden on society and its economics, will thus increase.In responding to this practical need, several private long-term care facilities have been established in sequence for the past three years. However, their quality varies which poses a difficult decision-making for the patient and family. This study was conducted to evaluate thirteen private long-term care facilities in Kaohsiung area by on site visites. Its purpose was to provide a reference for referral to redice hospital loading and to meet the urgent need of the patient and family. The evaluation comprised five major components: basic data, equipment used, staffing, rehabilitation services provided, and patient identifying information. Several problems were identified after one month of investigation: 1) None of them was accredited. 2) They were mostly stuffed by unqualified paramedicals 3) The usage rate of rehabilitation equipment was extremely low. 4) Most of them did not have enough space for activities. 5) The frequency of physician's ward round was either none or very few. The results of this study demonstrated that the existing private long-term care facilities in Kaohsiung area did not meet the requirements of chronic medical care. It is urged that the conditions of these private facilities be improved and put under medical management system, and be undertaken the establishment of standard public nursing facilities.
