Abstract

Spinal arteriovenous malformations is a rare disease. The initial signs are usually in the forms of nonspecific sensory symptoms such as pains and parasthesia; therefore, it is difficult to conclusively diagnose this disease. Here, we report the case of a 30-year-old woman who suffered from a sudden weakness in the 4 limbs 15 years ago. The symptoms subsided after supportive care; she was discharged from a local hospital without further follow ups. Approximately 5 years ago, she had a sudden attack of pains in her left shoulder. The symptoms rapidly progressed to paralysis of the 4 limbs. She was sent to the emergency room. Magnetic resonance imaging (MRI), magnetic resonance angiography (MRA), and angiographic studies revealed intramedullary arteriovenous malformations at the 6th, 7th level of cervical spinal cord; consequently a trans-arterial embolization was performed. However, after 5 years of follow-ups and rehabilitation, her motor, sensory, and sphincter functions were still poor. She could not perform her daily living activities independently.This type of spinal arteriovenous malformations occurs in relatively younger populations, and it is localized at higher levels of the spinal cord. The malformations is difficult to treat because of the deep anatomic location and the rapid progression of symptoms. Due to the poor prognosis of neurological recovery, rehabilitation programs with a focuse on the compensation technique are recommended.
