Abstract

One hundred and thirty-four babies who were clinically diagnosed with postural wry neck were enrolled in this study. All of them received sonographic examination of both sternocleidomastoid muscles with a 7.5 MHz linear array transducer connected to the Acuson 128 10M ultrasonographic machine. Three (2.2%) of the 134 babies were noted to have echogenic changes in one of their sternocleidomastoid muscles, and were diagnosed as having muscular type torticollis. Among the other 131 babies, 116(88.6%) babies showed no difference between bilateral SCM muscles. The direction of the head tilt was not related to the thickness of the muscle, which indicated that the cause of the wry neck in these babies was not related to the SCM muscle itself. Fifteen (11.2%) babies revealed mild hypertrophy on one side, and the majority (80%) of these babies tilted their head in the direction of the hypertrophic muscle. The cause of the mild hypertrophy was not determined in this study. Whether the minimal muscle change or the hypertrophy was due to the result of operative error is worth further investigation. We concluded that, when evaluating the SCM muscle with ultrasonography, the echogenecity of the muscle is the most important finding, and the muscle thickness is also a helpful parameter. Ultrasonography is also useful to confirm the diagnosis of postural wry neck, and it helps to reduce the false negative rate of detecting the muscular type torticollis.
